Please read the following carefully, initial where noted, and sign below.

(Client’s initials) I agree, understand, and acknowledge that the elimination or modification of behaviors cannot be guaranteed. I acknowledge and understand that because of differences in the mental make-up and various characteristics of dogs, training will not provide exact results for every dog. Each dog is different in regard to ability, breeding, and temperament, therefor predicting the outcome is impossible. I further agree to accept responsibility for any and all damages the above-named dog may cause to persons and/or property through malicious, aggressive or improper behavior that may occur before, during, or after training provided by A Crate Escape, LLC.

(Client’s Initials) I understand and agree that dog training may involve risks to me, my family members or others and/or my dog. I hereby assume all risks associated with participating in this training and will not hold A Crate Escape, LLC, it or its instructors responsible in the event of injury to me, my family members or others and/or Dog. I also agree to assume all responsibility for any damage done to property, persons, or other dogs done by Client or Dog's actions.

(Client’s initials) My initials indicate that I agree and understand that the above-named dog participating in the training sessions is free of any infectious disease and is current on all appropriate vaccinations, including Bordetella, Distemper, Parvovirus and Rabies. I also understand that a verification of all shots must be provided before above named dog may participate in any training sessions. I also understand that all dogs enrolled in the training sessions must receive monthly preventative treatments for the control of fleas. If live fleas are found on dog, we will apply a topical product or give an oral treatment at owner’s expense.

(Client’s initials) My initials indicate that I understand A Crate Escape, LLC can use my pet's photograph(s) and/or video(s) taken during training sessions for educational and promotional purposes in any type of media.

At Trainer's sole election, Trainer's duties hereunder shall terminate if (a) in Trainer's sole judgment Dog is dangerous or vicious toward Trainer or any other person or animal, or interferes with the training of other dogs, or (b) Client breaches any term or condition of this Agreement. Upon termination in accordance with the foregoing, Trainer's duties shall terminate but all other provisions of this Agreement shall continue in full force and effect.

This Agreement is binding upon Client, spouse of Client, and children of Client. This Agreement supersedes all prior discussions, representations, warranties and agreements of the parties, and expresses the entire agreement between Client and Trainer regarding the matters described above.

HOLD HARMLESS: The client will hold A Crate Escape’s owner(s), employee(s), and agent(s) free and harmless from any and all claims, liabilities, damages, losses, or expenses arising out of any injury to any person or to any person’s property by the Dog(s) named in this contract while in the care, custody, and control of A Crate Escape’s owner(s), employee(s), or agent(s).
